What are the five D's of neck pain?

Triple Crown Chiropractic · Published November 4, 2026 · 2 min read

Know the signs. Know when to act.

The “five D’s” is a traditional warning-sign mnemonic: dizziness, diplopia (double vision), dysarthria (difficulty speaking), dysphagia (difficulty swallowing) and drop attacks (sudden unexplained falls). New symptoms like these with neck pain can signal a serious neurological or vascular problem. The list is not a diagnostic test or a way to clear someone for manipulation.

Let’s look at what matters

Understand the terms without self-diagnosing

Dizziness has many possible causes and is not, by itself, proof of a stroke. Double vision, slurred speech, swallowing difficulty or sudden unexplained collapse also have multiple causes. Their onset, combination and associated symptoms matter.

The mnemonic is incomplete. Facial droop, one-sided weakness, loss of coordination or sudden severe unusual headache may be important even when none of the five labels seems to fit.

What to do when symptoms are new

Call 911 for sudden stroke-like symptoms, including sudden speech or vision changes, weakness or loss of balance. Note when symptoms started, do not drive yourself and do not wait for a chiropractic appointment. Symptoms that resolve can still require emergency assessment.

Do not try to reproduce symptoms by rotating or extending your neck. An online screen cannot establish that neck manipulation is safe, and a negative self-test is not reassurance.

What this means in Walton and Covington

For residents of Walton, Covington and surrounding Northern Kentucky communities, this is a safety question—not a reason to choose an office across town. If symptoms occur while commuting toward Cincinnati, stop safely and call emergency services. Discuss non-emergency concerns with Dr. Erik Simms only after urgent conditions have been addressed.

Explore our Walton office and Covington office for current hours, directions and contact details. These are our two offices; nearby communities are service areas, not additional locations.

Practical next steps

  • Treat sudden neurological changes as urgent.
  • Record the symptom onset time if possible.
  • Do not seek an adjustment to test whether the symptoms disappear.
